Le sujet d\u00e9range, parfois irrite, souvent inqui\u00e8te. Pourtant, l\u2019\u00e9viter serait une erreur. Car l\u2019absence n\u2019est pas qu\u2019un indicateur: c\u2019est un signal. Le chiffre frappe, mais il ne dit pas l\u2019essentiel: les plannings qui se d\u00e9font \u00e0 l\u2019aube, les r\u00e9organisations de derni\u00e8re minute, la charge qui glisse vers les pr\u00e9sent, et, en toile de fond, la qualit\u00e9 et la s\u00e9curit\u00e9 des soins qu\u2019il faut pr\u00e9server sans compromis. <\/p>\n\n\n<h2 class=\"highlight\">\nEn 2024, nos projections \u00e9voquaient une d\u00e9rive possible jusqu\u2019\u00e0 9,5 %. Nous avons refus\u00e9 de consid\u00e9rer cette courbe comme une fatalit\u00e9.<\/h2>\n\n\n<h2 class=\"wp-block-heading\">Assumer un mod\u00e8le d\u00e9centralis\u00e9<\/h2>\n\n\n\n<p>Nous nous sommes pos\u00e9 des questions simples et structurantes. Fallait il lancer un programme global, uniforme, cens\u00e9 s\u2019appliquer \u00e0 tous? Devions nous renforcer les r\u00e8gles, centraliser davantage, standardiser les r\u00e9ponses? Nous avons observ\u00e9 d\u2019autres mod\u00e8les, y compris tr\u00e8s centralis\u00e9s, o\u00f9 les d\u00e9cisions RH fa\u00e7onnent les \u00e9quipes \u00e0 distance du terrain. Nous avons compris que ce n\u2019\u00e9tait pas notre voie.<\/p>\n\n\n\n<p>Notre choix a \u00e9t\u00e9 clair: assumer un mod\u00e8le d\u00e9centralis\u00e9, confiant dans les cadres de proximit\u00e9. Le cadre recrute, construit son \u00e9quipe, pilote, r\u00e9gule, d\u00e9cide. Les RH soutiennent, s\u00e9curisent, conseillent, accompagnent. Cette coh\u00e9rence est d\u00e9terminante. L\u2019absence se g\u00e8re l\u00e0 o\u00f9 elle se vit, au plus pr\u00e8s des r\u00e9alit\u00e9s humaines et organisationnelles. <\/p>\n\n\n\n<p>Ce chemin a exig\u00e9 de l\u2019humilit\u00e9. Nous avons confront\u00e9 nos pratiques, \u00e9chang\u00e9 avec d\u2019autres h\u00f4pitaux, accept\u00e9 de revisiter certaines certitudes. Ce qui change les dynamiques, ce sont des actes manag\u00e9riaux concrets.\u00bb<\/figcaption><\/figure>\n<\/div>\n\n\n<h2 class=\"wp-block-heading\">Trois constats marquants<\/h2>\n\n\n\n<p><strong>Premier constat: <\/strong>les grandes campagnes institutionnelles rassurent, mais ne transforment pas toujours durablement. Ce qui change les dynamiques, ce sont des actes manag\u00e9riaux concrets, r\u00e9p\u00e9t\u00e9s, visibles. Dans une unit\u00e9 o\u00f9 les absences courtes culminaient en d\u00e9but de semaine, la reprise des plannings, la clarification des attentes et la syst\u00e9matisation des retours ont suffi \u00e0 infl\u00e9chir la tendance. Pas d\u2019effet d\u2019annonce, mais de la coh\u00e9rence et de l\u2019\u00e9quit\u00e9 per\u00e7ue.<\/p>\n\n\n\n<p><strong>Deuxi\u00e8me constat:<\/strong> l\u2019entretien de retour ne se r\u00e9sume pas \u00e0 une simple formalit\u00e9 administrative. C\u2019est un moment cl\u00e9 de management. Lorsqu\u2019il dispara\u00eet, l\u2019absence se banalise. Lorsqu\u2019il est men\u00e9 avec justesse, il restaure le lien et redonne du sens. <\/p>\n\n\n<h2 class=\"highlight\">\nDans un service sous tension, une succession d\u2019absences r\u00e9currentes r\u00e9v\u00e9lait une difficult\u00e9 d\u2019int\u00e9gration et un malaise d\u2019\u00e9quipe. Le dialogue, appuy\u00e9 par les RH, a permis d\u2019identifier la cause r\u00e9elle et d\u2019agir de mani\u00e8re cibl\u00e9e.<\/h2>\n\n\n<p><strong>Troisi\u00e8me constat:<\/strong> l\u2019anticipation vaut mieux que la r\u00e9action. Nous avons instaur\u00e9 des \u00e9changes r\u00e9guliers entre cadres, RH et m\u00e9decine du personnel. Non pour contr\u00f4ler, mais pour r\u00e9fl\u00e9chir et d\u00e9cider plus t\u00f4t. Le taux s\u2019est stabilis\u00e9 \u00e0 8 %. Ce n\u2019est ni un exploit ni un aboutissement. C\u2019est un point de d\u00e9part. La d\u00e9monstration qu\u2019une mobilisation collective peut infl\u00e9chir une trajectoire.<\/p>\n\n\n<h2 class=\"highlight\">\nLes effets organisationnels sont tangibles. Une meilleure anticipation des absences a permis de r\u00e9duire de moiti\u00e9 le recours \u00e0 l\u2019int\u00e9rim. <\/h2>\n\n\n<p>Moins de discontinuit\u00e9, plus de stabilit\u00e9, moins d\u2019\u00e9nergie d\u00e9pens\u00e9e \u00e0 r\u00e9expliquer dans l\u2019urgence. Le b\u00e9n\u00e9fice est imm\u00e9diat pour les \u00e9quipes comme pour les patient\u00b7e\u00b7s.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Lib\u00e9rer du temps pour la relation humaine<\/h2>\n\n\n\n<p>Nous avons \u00e9galement renforc\u00e9 l\u2019alignement manag\u00e9rial. Un s\u00e9minaire d\u00e9di\u00e9 a permis de partager des rep\u00e8res communs: conduire un entretien de retour, documenter sans rigidifier, maintenir une posture \u00e9quilibr\u00e9e entre \u00e9coute et exigence. L\u2019enjeu est clair: passer d\u2019une gestion r\u00e9active \u00e0 une posture proactive, coh\u00e9rente et \u00e9quitable. Enfin, un levier souvent sous estim\u00e9 concerne la charge administrative. Quand le cadre consacre ses soir\u00e9es \u00e0 consolider des tableaux, il perd du temps de pr\u00e9sence manag\u00e9riale. Nous exp\u00e9rimentons donc des outils num\u00e9riques pragmatiques: comptes rendus automatis\u00e9s, tableaux de bord simplifi\u00e9s, alertes sur les signaux faibles.
