Cette \u00e9volution met en lumi\u00e8re un premier paradoxe du syst\u00e8me de sant\u00e9: plus les maladies sont identifi\u00e9es, meilleure est la prise en charge, mais plus les co\u00fbts et la complexit\u00e9 organisationnelle augmentent. Les h\u00f4pitaux doivent se sp\u00e9cialiser et renforcer leurs interconnexions, ce qui complexifie leurs structures. Parall\u00e8lement, un second paradoxe appara\u00eet: l\u2019h\u00f4pital est organis\u00e9 de mani\u00e8re hi\u00e9rarchique, alors que ses logiques centrales (m\u00e9dicales, soignantes, administratives et \u00e9conomiques) ne peuvent \u00eatre hi\u00e9rarchis\u00e9es. <\/p>\n\n\n<h2 class=\"highlight\">\nLes d\u00e9cisions m\u00e9dicales n\u2019ob\u00e9issent pas \u00e0 une logique administrative, pas plus que les co\u00fbts ne peuvent \u00eatre pilot\u00e9s selon des crit\u00e8res cliniques.<\/h2>\n\n\n<p>L\u2019innovation \u00e0 l\u2019h\u00f4pital ne peut donc pas \u00eatre pens\u00e9e uniquement sous l\u2019angle m\u00e9dico-technique. Elle est aussi, et toujours, organisationnelle: nouveaux r\u00f4les, parcours de soins repens\u00e9s et formes in\u00e9dites de collaboration. Les formations au leadership offrent aux m\u00e9decins un espace prot\u00e9g\u00e9 pour analyser ces tensions, questionner leurs pratiques et relier perspectives m\u00e9dicales et organisationnelles.
