Selon l\u2019experte, les nombreux changements de direction observ\u00e9s dans les h\u00f4pitaux suisses s\u2019expliquent davantage par des transformations structurelles que par de v\u00e9ritables crises. Une crise est, par d\u00e9finition, soudaine et impr\u00e9vue. <\/p>\n\n\n<h2 class=\"highlight\">\nOr, le syst\u00e8me hospitalier subit une forte pression: les anciens mod\u00e8les de gouvernance ne suffisent plus. Un changement de direction est alors plus simple qu\u2019un renouvellement complet de l\u2019\u00e9quipe dirigeante.<\/p>\n\n\n\n<p>Cette instabilit\u00e9 cr\u00e9e de l\u2019incertitude et accentue la rotation du personnel. Apr\u00e8s chaque d\u00e9part, les \u00e9tablissements passent beaucoup de temps \u00e0 se r\u00e9organiser, ce qui entra\u00eene la perte de collaboratrices\u00b7teurs: un probl\u00e8me particuli\u00e8rement aigu dans un contexte de p\u00e9nurie de personnel qualifi\u00e9. <\/p>\n\n\n<h2 class=\"highlight\">\nLa communication, quant \u00e0 elle, joue un r\u00f4le cl\u00e9 dans ces processus, favorisant la confiance et la coop\u00e9ration.<\/h2>\n\n\n<p>Les petits h\u00f4pitaux peinent surtout \u00e0 absorber leurs co\u00fbts fixes \u00e9lev\u00e9s, tandis que les grands \u00e9tablissements doivent g\u00e9rer une complexit\u00e9 croissante. Par ailleurs, le r\u00f4le des cantons tend parfois \u00e0 fausser la concurrence. Selon Nanda Samimi, il faudrait int\u00e9grer syst\u00e9matiquement le rench\u00e9rissement dans les tarifs.
